Palm Checkout Acknowledgement Form

Background information:

Each sophomore student has the opportunity to checkout a Palm M100 unit for use throughout their sophomore year at Norris. The Palm will be treated like a textbook checkout. The Palm will be utilized in a variety of ways throughout the curriculum. We feel that these devices will also serve as an excellent organization tool for your child.

Key Points:

	Students should only write on the Palm’s using a stylus. (Any other items will damage the screen.)
	Students should keep the snap-on cover on the unit at all times. This will protect the unit from damage.
	Just like a textbook…the student/parent is responsible for loss and/or damage.
	Units will be checked out via a number and serial number. All units will be checked in prior to the end of the school year.
	These units are for school use only.
	Encourage your child to take good care of their Palms and not to leave them unattended.
	Students will be responsible for maintaining batteries in the unit.
	Batteries, extra stylus, and extra covers will be available for purchase at the school.
	Palm M100 replacement cost is $100

Please sit down and look at the unit with your child. Visit with them about the responsibility they have for caring for the unit. This is an excellent opportunity for you and our school to reinforce the importance of responsibility.
